Rank: Check the Ranks topic. Every character starts out at the lowest possible level for their group. If you're a Noah, add what memory your character possesses.

Family: A list of your character's family or the people who are/were close to him/her.

Crushes: If your characters has a crush on someone... can also be their gender preference.

Abilities [Noahs and Akuma]/Innocence [Exorcists]: Write here the name of your characters Ability/Innocence, remember: ONLY THE NAME!

Personality: What is he/she like? Write at least 5 lines here!

Likes: Things and people he/she likes...

Dislikes: Things and people he/she doesn't like...

History: Your character's life, where was he/she born, what did happen to him/her etc... Write at least 10 lines here!

Height & Weight: (in centimeters and kilograms, if possible)

Looks/Picture: Describe what does your character look like or put here a picture, that looks exactly like your character, you're allowed to make a few changes though. Describe the changes with short notes (e.g.: His eyes are blue, not green.)!
